Mask & Separation Pledge: I guarantee that all guests in my registration will continuously wear filters/masks, undergo security checks, and comply with 6ft distance directives inside the temple or auditorium campus boundaries. *

Health Declaration Agreement: I explicitly testify that inside the preceding 14 days, neither I nor any registered dependent has registered temperatures exceeding 100.4 F, displayed throat or breathing symptoms, or shared spaces with a Covid positive individual. *
